The table for contributions is as follows:

Level 1: $1200 per year = $100 per month

Level 2: $2400 per year = $200 per month

Level 3: $4000 per year = $333.33 per month

Level 4: $7500 per year = $625 per month

Level 5: $10,000 per year = $1250 per month
